What to do When Your NDIS Plan is About to Expire

As your NDIS plan approaches its end date, it’s important to take steps early to ensure there is no disruption to your supports. Whether you’re a participant, carer, or support person, staying on top of plan expiry dates can make the transition to your next plan smooth and stress-free. Here’s a simple guide to help you prepare.

Know your Plan end date

The first step is to check when your current plan ends. This information is available on your NDIS portal (myplace), in your plan documents, or through your support coordinator or plan manager.

Tip: Set a reminder in your calendar at least 8–12 weeks before your plan is due to end so you can start preparing early.

Talk to your Support Coordinator

Your Support Coordinator is there to help you get ready. Reach out to them early to:

Discuss how your current plan has worked for you

Identify any gaps or challenges

Consider new goals or supports you might need

They can help you prepare for your plan review and ensure you have the right documentation and reports.

Gather updated Reports and Evidence

Having up-to-date assessments and reports is crucial for a successful plan review. Reach out to allied health professionals (like OTs, speech therapists, or psychologists) for updated progress notes and recommendations.

Helpful documents include:

  • Functional capacity assessments
  • GP or specialist letters
  • Progress reports from providers
  • Any new risk or safety concerns

Reflect on what’s worked and what hasn’t

Take some time to think about:

What supports have been helpful?

Are there any services you didn’t use?

Have your needs or circumstances changed?

This reflection helps your new plan better reflect your goals and current situation.

Book your Plan Review meeting

If you haven’t been contacted about a plan review, you can request one by:

Calling the NDIS directly on 1800 800 110

Speaking with your support coordinator

Make sure the review is scheduled well before your plan end date to allow time for any follow-up.

Ask about Plan Continuation

In some cases, the NDIA may extend your plan while you wait for a review. This ensures that funding continues without interruption. You can request a continuation or a new plan depending on your needs.

Stay calm and reach out for help

It’s normal to feel a little nervous about a plan review. But with preparation and the right support, the process can be smooth and empowering. If you’re unsure what to do, don’t hesitate to ask questions or get advice from your coordinator or trusted provider.

How can we help you?
